Eric Ferraiuolo 5925e3327a Remove ridiculous a:focus { outline: none; } rule

This rule should have never been here, it's one of the worst things we
could have done for accessibility :(

Fixed accessability mistake by removing a:focus {outline: none;} rule from buttons-core.css.

(!) .pure-help-inline has been replaced with .pure-form-message-inline. We still support the older classname but it is deprecated and will be going away in a future release. (Issue #32) (dannytatom)

Adding a classname before the responsive grid's unit classname now works. Before units were targeted using the ^= ("starts with") selector, which meant that if another classname preceded the unit classname, then the rule would not apply. (Issue #44)
